Change of & Access to Personal Data Forms
- Change of Data Request Form (legal name, legal gender and permanent address)
- Students may change all other addresses in Self-Service Banner.
- A student submitting a permanent address update must complete the Residency Analysis Form and attach the required supporting documentation as set forth in this Policy. A change of address in Banner will not automatically trigger this process.
- Medical school students should contact their respective Registrar for questions about data changes:
