  • Change of Data Request Form (legal name, legal gender and permanent address)
    • Students may change all other addresses in Self-Service Banner.
    • A student submitting a permanent address update must complete the Residency Analysis Form and attach the required supporting documentation as set forth in this Policy. A change of address in Banner will not automatically trigger this process.
