All degree-seeking students—on-campus or online—must complete the Wellness Center Requirements. Important due dates and instructions are listed below:

Attention International Students:
As of May 15, 2024, all international students must complete an IGRA blood test (Quantiferon Gold or TSpot) in the United States before starting their first semester if they were born in or traveled for more than one month to the countries listed below.

All students must also complete an Online Tuberculosis Questionnaire as part of the Wellness Center Requirements. If you answer yes to any screening questions, submit lab results from an IGRA blood test performed within 12 months prior to the start of classes.
Requirement Steps
Step 1: Required Health Forms

A. Complete Your Online Health Forms
Click here to complete your required health forms. Once logged in, select the green “Get Started” button under Rowan Medical Services & Wellness Requirements, then choose Wellness Center Requirements/Forms.
Complete the following required forms:
- Form 1: Personal Information Form
- Form 2: Personal & Family Health History Form
- Form 3: Meningitis Information Form
- Form 4: Tuberculosis (TB) Questionnaire Form
Important: After submitting each form, confirm that a “submit date” appears next to it in the portal.
B. Form 5: Print the Immunization Form and have your healthcare provider complete it.
Helpful tip: Immunization records may be obtained from:
- Your primary care provider or pediatrician
- Your high school or previous college/university
- Military records
- The New Jersey Immunization Information System (for students who attended school in NJ)
C. If under 18 years of age, print Form 6: Wellness Center Consent for Treatment of a Minor and have your legal guardian complete. All three pages must be completed and uploaded. Typed initials and signatures are not acceptable.

Step 2: Submitting Required Forms
A. Before uploading, review your documents to ensure all required forms are complete, signed, and dated where indicated.
Save each document as a separate file using one of the approved formats:
.gif, .png, .tiff, .tif, .jpg, .jpeg, .txt, or .pdf
File naming requirements:
- Do not use numbers or special characters
- Use clear, descriptive file names
B. Click here to upload your documents.
Once logged in, select the green “Get Started” button under Rowan Medical Services & Wellness Requirements, then choose Uploads and follow the on-screen instructions.
Upload your files under the appropriate categories in the Rowan University Wellness Portal:
- Immunization Record
- Consent for Treatment (students under 18 years old)
- Titer Lab Reports / Other Labs / Chest X-Ray Reports
(Includes any required titers, lab results, or radiology reports)

Step 3: Complete Online AOD Module (FRESHMEN ONLY)
All first-year students are required to complete the Alcohol and Other Drugs Online Module
- Time to complete: Approximately 35 minutes
- Login: Use your Rowan NetID and password
- Completion requirement:
- Complete all sections of the module
- Pass the final assessment with a score of 80% or higher

Student Health Insurance Waiver
Complete your student health insurance waiver. Note: student health insurance waivers are completed through the Bursar's Office.
NCAA Athlete Health Requirements
If you are a NCAA athlete, be sure to review the NCAA health requirements.
Your Medical Record
Your mandatory health forms will become part of your confidential medical record. The immunization form must be available for inspection by the state Department of Health. The state Epidemiologist requires an annual report of compliance to the immunization law. The Health Services staff will review your submitted forms. If any information is omitted or incomplete, or falls in an unacceptable time frame, you will be notified via email.
