Wellness Center Requirements
Incoming students! Your Wellness Center Requirements are due June 15th.

These requirements apply to ALL degree-seeking students, regardless of your campus location or online status.
*Please note: Rowan University no longer requires the COVID-19 vaccination, though vaccination is highly recommended. For more information regarding CDC recommendations click HERE.
Wellness Center requirement due dates:
ASCEND Program: June 15
Incoming Fall Students: June 15
Incoming Spring Students: December 15
Incoming Summer Students: June 15

Please note: As of May 15th, 2024, all international students are required to have an IGRA (Quantiferon Gold or TSpot) blood test performed in the United States prior to the beginning of their first semester if they were born in or have traveled for more than a month to any of the countries listed below. Additionally, every student must complete an Online Tuberculosis Questionnaire as a part of the Wellness Center Requirements. Students must submit the lab results from an IGRA blood test performed in the United States dated within 12 months prior to the start of classes if they answer yes to any of the screening questions.Requirement Steps
Step 1: Required Health Forms
- Form 1: Personal Information Form
- Form 2: Personal & Family Health History Form
- Form 3: Meningitis Information Form
- Form 4: Tuberculosis (TB) Questionnaire Form
**Review: you should notice a “submit date” next to each form once they have been
completed.
B. Form 5: Print Immunization Form for your healthcare provider to complete.
Helpful Tip: Immunization records can be obtained from your primary care provider (pediatrician), high school, another university/college or military records. Any official documentation of vaccines is acceptable (yellow book) etc. Additionally, if you lived in New Jersey while attending school, you may be able to obtain records from the New Jersey Immunization Information System.
C. If under 18 years of age, print Form 6: Wellness Center Consent for Treatment of a Minor and have your legal guardian complete. All three pages must be completed and uploaded. Typed initials and signatures are not acceptable.

Step 2: Submitting Required Forms
A. Once your documents are ready to be submitted, look them over to make sure all required forms are complete, signed, and dated where indicated. Then scan and save your documents into separate files in the categories listed below. Save your files in one of the following file types: .gif, .png, .tiff, .tif, .jpg, .jpeg, .txt or .pdf. Do not use any numbers or special characters in your file name.
- Immunization Record
- Consent for Treatment - Students under the age of 18 years old
- Titer Lab Reports/Other Labs/Chest X-Ray Reports – Any other required titers, labs and/or radiologist’s reports
