Injuries and Hazardous Substance Exposures Reporting:

For immediate medical assistance call 9-1-1.

A Rowan Incident Report Form must be completed for all injuries and hazardous substance exposures. Please refer to the Risk Management website for further information:

Laboratory Incident Report Form:

Use the Laboratory Incident Report Form for:

• Hazardous Substance Spills (Chemical, Biological, Radiation)
• Property Damage
• Uncontrolled Material Release
• Equipment Failure
• Failure to follow proper procedure
• Any other incident where a further investigation is needed to prevent reoccurrence.

This form is not for injuries or hazardous substance exposures (see above).
