Reporting Forms

The following forms are now web-based and can be found on the N.J. Ethics Commission website and selecting a Form located beneath the "Forms" Menu on the lefthand side of the page, or you may get common forms located here:

• Outside Activity Questionnaire (OAQ)
• Attendance at Events (AAE)
• Annual Disclosure Form 

The Annual Disclosure Form/Scholarly Capacity Disclosure Form, can be appended with information from every AAE done in a Scholarly Capacity.  These forms must be submitted to Ray Braeunig, Chief Audit, Compliance and Privacy Officer, no earlier than June 30th which is the end of the annual reporting period.
