Conflict of Interest (COI) and Significant Financial Interest (SFI) Disclosure Form

IMPORTANT: 09/23/2022 - The Investigator Financial and Other Personal Interest Form has been updated. The form is now titled: Conflict of Interest (COI) and Significant Financial Interest (SFI) Disclosure Form. Please use the new form for reporting Significant Financial Interests (SFI), Other Support and any Conflict of Interests (COI).

Conflict of Interest (COI) and Significant Financial Interest (SFI) Disclosure form must be completed for any grant or sponsored project whose funding agency (sponsor) is NSF or DHHS agency. The form may also be required to be completed for any award or subaward whose funding agency requires a review of conflict of interests and/or other support, including international support.

Rowan faculty and staff who participate in or receive in-kind support from non-US, foreign governments or entities owned by non-US, foreign governments should report the support in the Other Support section of the Investigator Financial and Other Personal Interest Form.

Conflict of Interest (COI) and Significant Financial Interest (SFI) Disclosure form is required for human subjects research studies submitted through the Rowan University Cayuse IRB/Human Research Ethics module. Attach completed COI/SFI form in the Rowan Cayuse IRB submission.

Conflict of Interest (COI) and Significant Financial Interest (SFI) Disclosure form is required for any Rowan University Cayuse Sponsored Programs proposal that indicates "Yes" that a Principal Investigator, (PI) Co-PI, Co-Investigator or key researcher/personnel on the proposed research project has a COI and/or SFI. Submit completed COI/SFI form to the Office of Research Compliance Conflict of Interest Committe.
